You may already be a Care Partner for a parent, spouse or relative. From the smallest chores - like getting a newspaper or picking up a prescription - to the biggest challenges – such as managing finances or administering medications - YOU are involved and committed to helping your loved one.
Whether you are just beginning to care for someone or have been doing so for years, we recognize that your involvement may positively influence the success of their medical care, decrease their length of stay in the hospital and promote fewer avoidable re-admissions.
Southide Regional Medical Center is pleased to invite you to become a Care Partner while your loved one is in the hospital. Patients may have two (2) Care Partners per admission. Care Partners will receive a Care Partner badge which must be worn while in the hospital. This badge allows Care Partners to come and go as needed and it identifies you as a member of the treatment team.
If you would like to become a Care Partner during your loved one’s hospital visit at Southside Regional Medical Center, call 804-765-5683. Please leave a message with your name, phone number and the best time to reach you. We will schedule a time to meet you and the patient in their room to review the program. You will be introduced to the patient’s nurse and be given a special tour of the nursing unit. Specific instructions, training and education will be provided to help you feel more comfortable and confident while caring for your loved one.
Remember, we believe Care Partners are members of our health care team and share the right to ask questions, express concerns and make suggestions. Your participation is welcomed and appreciated!
