The Health Information Management (HIM) Department and Your Medical Records

To obtain copies of your medical records come to the Health Information Management Department located on the first floor next to Administration. Please go to the Release of Information window to be assisted.

Hours of Operation:
Monday through Friday 8:00 a.m. through 4:30 p.m.
Closed on Saturday, Sunday, and Holidays

All requests must be accompanied by an authorization form that has been completely filled out and signed along with photo identification. For your convenience the form is available on this website and can be downloaded and completed prior to coming. Click here to download the PDF file.

The following fees may be applicable:

• $0.50 per page for the first 50 pages and
• $0.25 per page thereafter
