Please complete the TEAS Test Request Form and email it as an attachment to Erica Wells, Admission Officer, at Erica_Wells@chs.net. Upon receipt of your TEAS Test Request Form, a confirmation email will be sent to you. Please be sure to select two (2) preferred testing dates.

Location:
The TEAS is currently being administered in the Southside Regional Medical Center (SRMC) computer lab, which is located on the first floor of the hospital. SRMC is located at 200 Medical Park Boulevard, Petersburg, VA 23805.

Time:
TEAS will begin at 8:00 a.m. No one will be granted admission into the hospital computer lab after 8:15 a.m. No exceptions will be made.

Fees:
ATI charges a $35 fee that must be paid by credit/debit card. SRMC Professional Schools also charges a $10 proctoring fee that must be paid by cash or check upon entrance to take the exam.
