Welcome to Transitions, Southside Regional Medical Center's Skilled Nursing Unit.  Transitions provide the link between inpatient hospital treatment and your next treatment phase, whether your treatment will continue at home or at another facility.

Discharge planning for patients admitted to Transitions begins early in your hospital stay.  Your treatment team, which typically includes your physician, a physical therapist, respiratory therapist, social worker and others, will meet with you and your family individually, or as a group, to discuss your treatment goals and progress.

The level of care you will receive on the Transitions unit is different than the level of care you have experienced in a hospital.  One of the treatment goals for patients and their families is to educate the patient and their caregiver as to how they may assist their loved one with their care, daily hygiene, and mobility.  Educating the patient and their caregiver during this phase of treatment leads to a smooth transition to home or other treatment facility.
